PROOF OF RESIDENCY: Please see the attached list of acceptable documents, containing the full residence of parent(s) or legal guardian(s) dated between February 1 of the previous year and February 1 of the current year: LL Residency Requirements

PROOF OF SCHOOL ENROLLMENT: The attached school enrollment form is only required to be completed once during the participant's career, unless school enrollment changes: School Enrollment Form *League ID# 151139* (For PW7 & PW8, please write in the level.)
